Overview of AI Generation
The AI system analyzes all available patient data to create structured handoff notes following best practices and clinical standards.Comprehensive
Includes all relevant patient information
Structured
Follows SBAR format for clarity
Intelligent
Highlights critical information
Generation Methods
There are two primary methods for generating handoff notes:Method 1: Generate from Patient Record
This method uses structured patient data from your EHR system. The button displays a sparkles icon and will show “Generating…” with a loading spinner during processing.
Demographics
Demographics
- Patient name, age, and sex
- Medical record number (MRN)
- Room number
- Code status
Clinical Information
Clinical Information
- Chief complaint or admission reason
- Current diagnosis
- Medical history
- Allergies and medication sensitivities
Vital Signs
Vital Signs
- Latest temperature reading
- Heart rate and rhythm
- Blood pressure measurements
- Respiratory rate
- Oxygen saturation (SpO2)
Medications
Medications
- Current medication list
- Dosages and routes
- Administration schedule
- Recent medication changes
Task Status
Task Status
- Tasks completed during current shift
- Outstanding tasks requiring attention
- Priority levels for pending tasks
- Scheduled task times
Previous Notes
Previous Notes
- Previous handoff notes for continuity
- Significant changes since last handoff
- Ongoing concerns or issues
A loading indicator with “Generating…” text appears during processing. You can continue viewing patient information while generation completes.
Method 2: Upload and Analyze Handoff Image
This method uses OCR and AI to extract information from images of handoff documents.- JPEG/JPG
- PNG
- HEIC (iPhone photos)
- WebP
Ensure images are clear and well-lit. Blurry or low-resolution images may result in inaccurate text extraction.
Review the preview to ensure the image is oriented correctly and all text is legible before proceeding.
- Extract text from the image using OCR
- Interpret medical terminology and abbreviations
- Identify key clinical information
- Structure the extracted data
- Information from the image
- Current patient record data
- Synthesized analysis from both sources
The system displays: “Image analyzed! Regenerating integrated handoff notes…” followed by “Image analyzed and integrated into handoff notes!” when complete.

Generated Note Structure
AI-generated handoff notes typically follow the SBAR format (Situation, Background, Assessment, Recommendation):SBAR Framework
- Situation
- Background
- Assessment
- Recommendation
Current patient status and immediate concernsIncludes:
- Patient identification (name, age, MRN, room)
- Current diagnosis or chief complaint
- Code status
- Key alerts (allergies, fall risk, isolation precautions)
Formatting Features
Generated notes include:Section Headers
Clear headers separate different sections for easy scanning
Bullet Points
Key information presented in easy-to-read lists
Timestamps
Scheduled tasks include specific times
Priority Indicators
Critical items are highlighted or marked urgent
Regenerating Notes
You can regenerate handoff notes at any time:When to Regenerate
Patient status changes
Patient status changes
Significant changes in condition, vital signs, or treatment plan warrant regeneration to ensure accurate handoff.
Tasks completed
Tasks completed
After completing multiple tasks, regenerate to update the outstanding tasks section.
New information available
New information available
New test results, provider orders, or consultation notes should be incorporated.
Shift transition approaching
Shift transition approaching
Regenerate 30-45 minutes before shift end for the most current information.
Regeneration Process
AI Analyzes Current State
The AI will:
- Analyze all current patient data
- Compare with previous notes
- Identify what has changed
- Incorporate updates seamlessly
Previous Notes Preserved
Previous notes provide context:
- Continuity maintained
- Significant changes highlighted
- Historical context considered
Previous handoff notes are included in the AI prompt to maintain continuity and highlight changes since last generation.
Best Practices
Update Tasks First
Mark tasks as completed before generating notes to ensure accuracy
Verify Vitals
Ensure latest vital signs are documented in the system
Review for Accuracy
Always review generated notes for accuracy and completeness
Edit as Needed
Customize generated notes to add context or clarify information
Generate Early
Start generation 30-45 minutes before shift end to allow review time
Use Image Upload
Supplement with images when written notes contain information not in EHR
Troubleshooting
Generation fails or times out
Generation fails or times out
Possible causes:
- Network connectivity issues
- AI service temporarily unavailable
- Patient record missing required data
- Check network connection
- Wait 30 seconds and try again
- Verify patient record has required fields populated
- Contact IT if issue persists
Notes are incomplete or inaccurate
Notes are incomplete or inaccurate
Possible causes:
- Missing data in patient record
- Outdated information in EHR
- AI misinterpretation of clinical data
- Verify all patient data is current in the system
- Use the Edit function to correct or add information
- Regenerate after updating patient record
- Report systematic issues to your administrator
Image analysis fails
Image analysis fails
Possible causes:
- Image quality too poor
- File format not supported
- File size too large
- Text in image is illegible
- Ensure image is clear, well-lit, and in focus
- Use JPEG or PNG format
- Reduce file size if over 10MB
- Retake photo with better lighting/focus
- Manually type information if OCR continues to fail
Notes not saving
Notes not saving
Possible causes:
- Database connectivity issue
- Insufficient permissions
- Patient record locked by another user
- Error message will display if save fails
- Check network connection
- Try clicking Save again
- Verify you have write permissions
- Contact IT if issue persists
Generation Speed and Performance
Expected Timing
5-8 seconds - Basic patient record with standard information
10-15 seconds - Extensive medical history, multiple medications, many tasks
10-20 seconds - Depends on image clarity and text density
15-25 seconds - Image analysis + record integration + note generation
Performance Factors
- Patient Complexity
- System Load
- AI Service
- Number of medications
- Length of medical history
- Number of active tasks
- Amount of previous notes
Privacy and Security
Security Measures:- All data transmitted over encrypted connections (HTTPS/TLS)
- Anthropic is HIPAA-compliant and provides Business Associate Agreements (BAA)
- Patient identifiers are handled securely
- Access controlled by role-based permissions
- Audit logs track all access and modifications
