Generation Methods

There are two primary methods for generating handoff notes:

Method 1: Generate from Patient Record

This method uses structured patient data from your EHR system.
1
Start Generation
2
  • Navigate to a patient’s detail view
  • Locate the “Generate Handoff Notes” section in the right panel
  • Click the “Generate from Record” button
  • 3
The button displays a sparkles icon and will show “Generating…” with a loading spinner during processing.
    4
    Data Sources Analyzed
    5
The AI analyzes the following patient data:
    6
    • Patient name, age, and sex
    • Medical record number (MRN)
    • Room number
    • Code status
    • Chief complaint or admission reason
    • Current diagnosis
    • Medical history
    • Allergies and medication sensitivities
    • Latest temperature reading
    • Heart rate and rhythm
    • Blood pressure measurements
    • Respiratory rate
    • Oxygen saturation (SpO2)
    • Current medication list
    • Dosages and routes
    • Administration schedule
    • Recent medication changes
    • Tasks completed during current shift
    • Outstanding tasks requiring attention
    • Priority levels for pending tasks
    • Scheduled task times
    • Previous handoff notes for continuity
    • Significant changes since last handoff
    • Ongoing concerns or issues
    7
    Processing Time
    8
Generation typically takes 5-15 seconds depending on:
    9
  • Complexity of the patient record
  • Amount of historical data
  • Current server load
  • AI service availability
  • 10
A loading indicator with “Generating…” text appears during processing. You can continue viewing patient information while generation completes.
    11
    Auto-Save
    12
Generated notes are automatically saved to the database upon completion:
    13
  • No manual save action required
  • Notes are immediately available to all users
  • Timestamp records when notes were generated
  • Previous versions are retained for audit purposes
  • 14
The system displays a success message confirming “Handoff notes generated and saved successfully!” when complete.

Method 2: Upload and Analyze Handoff Image

This method uses OCR and AI to extract information from images of handoff documents.
    1
    Upload an Image
    2
  • Click the “Upload Handoff Image” button (camera icon)
  • Select an image file from your device
  • Supported formats:
    • JPEG/JPG
    • PNG
    • HEIC (iPhone photos)
    • WebP
  • 3
    Ideal Image Sources:
    4
  • Photos of handwritten handoff notes
  • Scanned printed reports
  • Pictures of whiteboard documentation
  • Digital screenshots of external systems
  • 5
Ensure images are clear and well-lit. Blurry or low-resolution images may result in inaccurate text extraction.
    6
    Preview and Verify
    7
    After upload:
    8
  • Image preview appears showing your uploaded file
  • Small “X” button in corner allows you to remove and try again
  • File name and size may be displayed
  • 9
Review the preview to ensure the image is oriented correctly and all text is legible before proceeding.
    10
    Analyze Image
    11
  • Click “Analyze & Add to Notes” button
  • The AI will:
    • Extract text from the image using OCR
    • Interpret medical terminology and abbreviations
    • Identify key clinical information
    • Structure the extracted data
  • 12
    Analysis Processing:
    13
  • Takes 10-20 seconds for typical handoff images
  • Loading spinner with “Analyzing…” text displays progress
  • More complex images may take longer
  • 14
    Integration with Patient Record
    15
    After image analysis:
    16
  • Extracted information is combined with existing patient record data
  • A new comprehensive handoff note is automatically generated
  • The integrated note includes:
    • Information from the image
    • Current patient record data
    • Synthesized analysis from both sources
  • 17
The system displays: “Image analyzed! Regenerating integrated handoff notes…” followed by “Image analyzed and integrated into handoff notes!” when complete.
    18
    Clear Image
    19
    To remove an uploaded image:
    20
  • Click the “X” button on the image preview
  • This removes the image but preserves any previously generated notes
  • Upload a different image if needed

    Generated Note Structure

AI-generated handoff notes typically follow the SBAR format (Situation, Background, Assessment, Recommendation):

    SBAR Framework

Current patient status and immediate concernsIncludes:
    • Patient identification (name, age, MRN, room)
    • Current diagnosis or chief complaint
    • Code status
    • Key alerts (allergies, fall risk, isolation precautions)
    Example:
    Patient: John Smith, 72-year-old male
    Room: 312B | MRN: 123456789
    Diagnosis: Acute exacerbation of CHF
    Code Status: Full Code
    Allergies: Penicillin (rash)

    Formatting Features

    Generated notes include:

    Section Headers

Clear headers separate different sections for easy scanning

    Bullet Points

Key information presented in easy-to-read lists

    Timestamps

    Scheduled tasks include specific times

    Priority Indicators

Critical items are highlighted or marked urgent

    Regenerating Notes

You can regenerate handoff notes at any time:

    When to Regenerate

Significant changes in condition, vital signs, or treatment plan warrant regeneration to ensure accurate handoff.
After completing multiple tasks, regenerate to update the outstanding tasks section.
New test results, provider orders, or consultation notes should be incorporated.
Regenerate 30-45 minutes before shift end for the most current information.

    Generate Early

Start generation 30-45 minutes before shift end to allow review time

    Use Image Upload

Supplement with images when written notes contain information not in EHR

    Troubleshooting

    Possible causes:
    • Network connectivity issues
    • AI service temporarily unavailable
    • Patient record missing required data
    Solutions:
    1. Check network connection
    2. Wait 30 seconds and try again
    3. Verify patient record has required fields populated
    4. Contact IT if issue persists
    Possible causes:
    • Missing data in patient record
    • Outdated information in EHR
    • AI misinterpretation of clinical data
    Solutions:
    1. Verify all patient data is current in the system
    2. Use the Edit function to correct or add information
    3. Regenerate after updating patient record
    4. Report systematic issues to your administrator
    Possible causes:
    • Image quality too poor
    • File format not supported
    • File size too large
    • Text in image is illegible
    Solutions:
    1. Ensure image is clear, well-lit, and in focus
    2. Use JPEG or PNG format
    3. Reduce file size if over 10MB
    4. Retake photo with better lighting/focus
    5. Manually type information if OCR continues to fail
    Possible causes:
    • Database connectivity issue
    • Insufficient permissions
    • Patient record locked by another user
    Solutions:
    1. Error message will display if save fails
    2. Check network connection
    3. Try clicking Save again
    4. Verify you have write permissions
    5. Contact IT if issue persists
Notes are automatically saved after generation, so manual save failures are rare.
