This guide walks you through the complete patient handoff workflow in the Nurse Handoff Helper application, from accessing the dashboard to generating and saving handoff notes.

Overview

The patient handoff process follows these main stages:
  1. Dashboard Access - View all active patients and their risk levels
  2. Patient Selection - Choose a patient to create handoff notes
  3. Information Review - Review patient details, vitals, and tasks
  4. Note Generation - Generate AI-powered handoff notes
  5. Review and Edit - Review and customize the generated notes
  6. Save and Distribute - Save notes for the next shift

Step 1: Access the Handoff Dashboard

When you log in, you’ll see the main Handoff Dashboard with an overview of all active patients.

  • Active Rooms: Total number of occupied rooms with patients
  • Critical Count: Number of patients with critical risk level
  • High Risk Count: Number of patients with high risk level

    Step 2: Filter and Select a Patient

    Click the “My Patients” button to show only rooms assigned to you. This is particularly useful when you have multiple nurses on the same floor.
    Use the risk level dropdown to filter patients:
    • All Risk Levels - Show all patients
    • Critical - Life-threatening conditions requiring immediate attention
    • High - Serious conditions requiring close monitoring
    • Medium - Stable but requiring regular care
    • Low - Stable patients with minimal intervention needed

    Selecting a Patient:

  • Click on any patient room card to open their detailed view
  • Each card shows:
    • Patient name, age, and sex
    • Room number
    • Current diagnosis or chief complaint
    • Risk level indicator
    • Last vital signs
    • Assigned nurse

    Once you select a patient, you’ll see a comprehensive view with two columns:

    • Full name
    • Age and sex
    • Medical record number (MRN)
    • Room number

    Clinical Details

    • Chief complaint
    • Code status
    • Medical history
    • Current diagnosis

    Vital Signs

    • Temperature
    • Heart rate
    • Blood pressure
    • Respiratory rate
    • Oxygen saturation

    Task Status

    • Completed tasks this shift
    • Outstanding tasks
    • Task priorities
    • Scheduled task times

    Patient information updates in real-time. Any changes made by other nurses or systems are automatically reflected in your view.

    Step 4: Generate Handoff Notes

    The right column contains the handoff note generation tools.

    Generation Options:

    Generate from Record
    Click “Generate from Record” to create AI-powered handoff notes based on:
    • Patient demographics and medical history
    • Current chief complaint and diagnosis
    • Latest vital signs
    • Medications and allergies
    • Task status (completed and outstanding)
    • Previous handoff notes (for continuity)
    The AI will synthesize all this information into a structured handoff note following SBAR (Situation, Background, Assessment, Recommendation) format.
    Upload Handoff Image
    Click “Upload Handoff Image” if you have a photo of:
    • Handwritten handoff notes
    • Printed reports
    • Whiteboard notes
    • Other documentation
    After uploading:
    1. Review the image preview
    2. Click “Analyze & Add to Notes”
    3. The AI will extract text and integrate it with the patient record
    4. A new comprehensive handoff note will be generated

    The generation process takes 5-15 seconds depending on the complexity of the patient record. The notes are automatically saved once generated.

    Step 5: Review and Edit Notes

    After generation, review the handoff notes for accuracy and completeness.

    Step 6: Manage Tasks

    Before completing the handoff, ensure all tasks are properly marked:

  • Tasks are organized into:
    • Completed This Shift - Tasks you’ve finished
    • Outstanding Tasks - Tasks that need to be handed off
  • Outstanding tasks are sorted by priority (critical → high → medium → low)

    Start Early

    Begin generating handoff notes 30-45 minutes before shift end to allow time for review and updates.

    Use Filters

    Filter by “My Patients” to focus on your assigned rooms and ensure you don’t miss anyone.

    Prioritize Critical

    Review critical and high-risk patients first to ensure comprehensive handoff of complex cases.

    Document as You Go

    Update tasks throughout your shift so handoff notes accurately reflect completed work.

    Real-Time Collaboration

    The Nurse Handoff Helper supports multiple nurses working simultaneously:
    • Live Updates: Changes made by one user are immediately visible to others
    • No Conflicts: The system handles concurrent edits gracefully
    • Activity Tracking: See when patient information was last updated
